What is the best way to manage pain in advanced cancer – morphine, fentanyl or something else?

Dear all, my sister has cancer bone (chondrasarcoma) in the basin for the past 4 years. The cancer is advanced and is now completely on painkillers. This a tumor growing slow growth in the basin and surrounding areas. She was taken Ultracet (37.5 acetaphenomin tramadol mg + 325 mg) for 2 years. Is currently about 5 tablets Ultacet day, but is still not getting relief from pain. She also takes anti-depressents. Now, doctors suggests starting drug than morphine / Duragesic. What is the good agreement between morphine and Duragesic (Fentanyl)? Your pain is cancer pain type (due to tumor growth and depresses nerves) and is there all day. Is there a better alternative, then morphine and Duragesic. Any aid would be greatly appreciated. Thank you & Best Regards, Naresh Agarwal

Fentanyl patches are probably the best. And they have a duration of action longer. Tramadol is not sufficiently strong to severe pain. And morphine should be administered every 4-6 hours and preferably intravenously.
